Membership application form.

Listed below are descriptions of the types of membership available.

Founding Member: A physician formally trained in pathology and either certified or recognised as a Pathologist in accordance to each country's regulations for specialists and who have demonstrated an interest and has contributed to the field of musculoskeletal pathology. Founder Members attended the Organizational Meeting held in Atlanta, Georgia, Sunday March 4th, 2001.

Member: Any physician formally trained in pathology and either recognised or certified in accordance with each country's regulation or customs for specialists and who has contributed to the field of musculoskeletal pathology.

Associate Member: Any physician or scientist who does not qualify as a regular member but who has demonstrated special interest in the field of musculoskeletal pathology, as evidenced by significant contributions.

Member In-Training: Any pathologist in training who has an interest in musculoskeletal pathology.

The following items must be submitted with this application prior to its evaluation by the Membership Committee:

1. An updated copy of your curriculum vitae, which should include biographical data, medical education, pathology education, specialty certification, academic and hospital positions (state current position first), membership in scientific organizations and bibliography.

2. A description of any special activities in musculoskeletal pathology, other than publications.

Please note that upon acceptance, the Treasurer will send a bill for dues.
